Bubble Form

Bubble Form, also commonly called Scantron Sheet, is simply another name for the machine-readable test or survey forms based on OMR (Optical Mark Read) technology that are widely used in education and industry. It is named for the characteristic fill-in circles-bubbles-that record users' answers to multiple-choice questions.
